Inova partners with ChartSpan to elevate chronic care management for patients
Fairfax, VA – March 26, 2024 – Inova Health System and ChartSpan, a leading provider of Chronic Care Management (CCM) solutions, announced a strategic partnership aimed at enhancing the delivery of chronic care management services to patients in Northern Virginia and the Washington, DC metropolitan area.
This collaborative effort combines ChartSpan’s dedicated Chronic Care Management (CCM) services with Inova’s renowned expertise in healthcare to offer patients a comprehensive and tailored approach to managing their health. Chronic Care Management ensures that Medicare patients with two or more chronic conditions receive continuous support and coordination of care, ultimately improving their quality of life and health outcomes.
Key elements of this partnership include:
- Seamless Integration: ChartSpan’s CCM services will be seamlessly integrated into Inova’s existing healthcare infrastructure, enabling efficient and secure data sharing and communication among healthcare providers, care coordinators, and patients.
- Personalized Care Plans: Patients with chronic conditions will benefit from personalized care plans, developed collaboratively by Inova’s skilled medical providers and ChartSpan’s care coordinators. CCM is now available at all Inova primary care locations.
