FALLS CHURCH, VA – April 22, 2015 – Inova’s Health Services Division announces the development of a new physician led network called Signature Partners. A clinically integrated network of physicians and hospitals in Northern Virginia, Signature Partners will work together to provide high quality, lower cost care by integrating and coordinating clinical services. In so doing, Signature Partners provides a high value network of providers for patients, employers and payers.
According to Dr. Matthew Poffenroth, Assistant Vice President, Inova and Chief Medical Officer for Signature Partners, this clinical integration model is leading the transformation of health care. “Clinical integration is the coordination of care delivery through collaboration and data sharing among physicians, hospital and other providers with common measures of quality and performance,” Poffenroth observed. “A clinically integrated network places the patient at the center of care and strives to deliver the right care, at the right time, by the right provider.”
“Signature Partners provides the foundation and infrastructure to achieve true clinical integration and care coordination for providers in our network and their patients”, Dr. Poffenroth added. Currently the Signature Partner network is growing and includes over 2,000 primary care and specialty care physicians. For more information on the Signature Partners Physician Network check out the website at www.signaturepartners.org or call 1-855-744-6895.
