Free sports physicals from Inova: July 16 & August 6, 2016

Inova Medical Group is pleased to bring two days of FREE sports physicals to our neighbors in Northern Virginia.

• Saturday, August 6

The physicals will take place at four Inova Primary Care locations (Annandale, Ballston, Gainesville, and Springfield).

Everyone who registers online for a sports physical will be entered to win an Ultra FitBit^® (one winner will be selected per day).

This is a standard Sports Physical, for individuals of any age. This sports physical does not replace a full, annual physical. Services performed include: height, weight, blood pressure, vision screening, and physical exam. A review of the patient’s medical history will also be done.

In lieu of payment, we are asking for a donation of non-pershiable food which we will collect and distribute to a local food bank serving the immediate community.

Virginia High School League requirement

If a student will be using our free sports physical to fulfill the VIRGINIA HIGH SCHOOL LEAGUE, INC. requirement, the student’s parent or guardian must fill out page 1 & 2 of the attached Athletic Participation/Parental Consent/Physical Examination Form
