In general, minimally invasive surgery refers to procedures performed using small incisions with the aid of specialized instruments such as tubes, cameras or endoscopes, rather than the larger incisions used in traditional open surgery. The goal of minimally invasive surgery is to minimize collateral tissue damage while achieving the same surgical objectives and outcomes as conventional approaches. By reducing trauma to surrounding structures, minimally invasive techniques typically offer patients faster recovery times, less postoperative pain, and shorter rehabilitation periods.
“With spine surgery in particular, technology is advancing to the point that outpatient surgery is possible, not only for smaller procedures such as laminectomies, but for larger surgeries like fusions and deformity corrections as well,” said Inova spine surgeon and minimally invasive expert Tan Chen, MD.
Over the past decade, technology has significantly advanced minimally invasive spine surgery. Modern imaging and navigation tools allow surgeons to see highly detailed, real-time views of a patient’s spinal anatomy. This improved visibility helps surgeons work with greater precision when placing screws, rods and other hardware, making procedures safer and more efficient.
While many of these technologies are currently more common in hospital settings due to cost, newer lightweight and more affordable systems are being developed. As a result, these advanced capabilities are increasingly becoming available in ambulatory surgery centers.
Another important innovation is expandable implant technology. Traditionally, minimally invasive procedures required smaller implants to fit through small surgical corridors. Now, some implants are designed to be inserted in a compact form and then expand once in position. This allows surgeons to achieve the strength and stability of larger implants used in traditional open surgery, while still preserving the benefits of a minimally invasive approach.

“Taking this technology a step further, patient-specific technologies offer the potential for 3D printed, custom implants designed specifically for that patient and based on their own anatomy,” Dr. Chen said.
Minimally invasive retractors are another tool surgeons use to reduce tissue disruption during spine surgery. Instead of cutting through muscle, these retractors gently separate and hold the muscle fibers apart, creating a pathway to the spine without detaching or damaging surrounding tissue.
By preserving the muscles, patients often experience faster recovery, less inflammation, reduced blood loss and less post-operative pain. Although this approach can be more technically demanding and offers a narrower working view for the surgeon, it can be highly effective in the right hands.
As Dr. Chen explains, “Minimally invasive surgery is like repairing a car engine through the tailpipe instead of lifting the hood, achieving the same result with far less disruption.”
Outpatient spine surgery, where patients return home the same day as their procedure, is becoming increasingly common, especially for smaller operations such as laminectomy or discectomy.
These procedures can be performed either in a hospital or in an ambulatory surgery center. One advantage of having outpatient surgery in a hospital is access to additional resources if something does not go as planned. If a patient needs extended monitoring, it is easier to stay overnight for observation when procedure is performed in hospital rather than at an offsite surgery center.
From a patient perspective, many people prefer to recover at home. Sleeping in a familiar, comfortable environment, without hospital noise or frequent overnight vital checks, can make experience less stressful. It is also important to check with your insurance provider, as shorter hospital stays and outpatient procedures are often more cost effective than longer inpatient stays.
“From a physician standpoint, it is critical to make an accurate diagnosis, identifying the source of a patient’s symptoms and developing a treatment plan that aligns with their goals.” Dr. Chen explains. “When the right spine procedure is performed on the right patient at the right time, the clinical results can be truly remarkable and life changing.”
Start your consultation with an open mind. Be honest about what you are experiencing and how your symptoms are affecting your daily life. Spine surgery is a significant decision, and it is essential that you feel comfortable with your surgeon. You should never feel dismissed or that your concerns are not being heard, regardless of how highly recommended a surgeon may be. If you need to seek more than one opinion to feel confident, that is completely appropriate.
One of the challenges in spine care is that there are often multiple ways to treat the same set of symptoms. It is commonly said that you can see five different spine surgeons and receive six different recommendations. The spine is complex, and treatment decisions are not always straightforward.
Ultimately, choosing the right path comes down to trust and communication. Do you feel confident in your surgeon’s expertise? Were your condition, treatment options and expected outcomes explained in a way that makes sense to you? Did your surgeon take time to answer your questions thoroughly? Feeling informed, respected and supported is just as important as the procedure itself.
