Inova Hospitals Earn ‘A’ Hospital Safety Grades from The Leapfrog Group

Two hospitals named Top Teaching Hospitals for Fall 2025 

Fairfax, VA – Inova is proud to announce that Inova Alexandria Hospital, Inova Fair Oaks Hospital, Inova Fairfax Hospital and Inova Loudoun Hospital have all earned an “A” Hospital Safety Grade for the fall term from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety. Leapfrog assigns an “A,” “B,” “C,” “D” or “F” grade to general hospitals across the country using evidence-based measures of patient safety focused exclusively on errors, accidents, injuries and infections. In addition, Inova Fair Oaks and Inova Loudoun hospitals were named Top Teaching Hospitals for 2025.

Key milestones in patient safety at Inova hospitals:

• Inova Loudoun Hospital is one of only of 11 hospitals nationwide and three in Virginia to earn “Straight A’s” since the program’s inception, marking an extraordinary 28 consecutive “A” grades over 14 years.
• Inova Fair Oaks Hospital has earned 27 consecutive “A” grades
• Inova Fairfax Hospital has maintained “A” grades for 15 consecutive grading periods.

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
