Inova hospitals Earn ‘A’ Hospital Safety Grades from The Leapfrog Group

Spring 2026 Safety Grade is a national recognition for commitment to patient safety

Fairfax, VA – May 6, 2026 – Inova is proud to announce that Inova Alexandria Hospital, Inova Fair Oaks Hospital, Inova Fairfax Hospital and Inova Loudoun Hospital have all earned an “A” Hospital Safety Grade for the spring term from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety. Leapfrog assigns an “A,” “B,” “C,” “D” or “F” grade to general hospitals across the country using evidence-based measures of patient safety focused exclusively on errors, accidents, injuries and infections.

Key milestones in patient safety at Inova hospitals include:

• Inova Loudoun Hospital is one of only of 11 hospitals in the U.S. and three in Virginia to earn 29 consecutive “A” grades since the program’s inception
• Inova Fair Oaks Hospital – 28 “straight A’s”
• Inova Fairfax Hospital – 16 “straight A’s”
• Inova Alexandria Hospital – 2 “straight A’s”

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
