Inova Loudoun Hospital Offering New Services on Leesburg Campus

Leesburg, VA – (October 23, 2012) — Inova Loudoun Hospital is now offering two new services on the Leesburg Campus.  The Inova Discharge Planning Clinic and the Inova Juniper Program are located at Inova Loudoun Medical Campus – Leesburg in the Jackson Building, 211 Gibson Street NW, Suite 206.  The new services occupy 1,875  square-feet of space consisting of four exam rooms and a therapy room. The office is open Monday through Friday, 8:30 a.m. to 5 p.m.

In July 2011 the Inova Discharge Planning Clinics were created to serve chronic diseases such as diabetes, COPD/pneumonia, CHF, and asthma. The vision for these clinics is to help transition patients who had recently been released from the hospital back into the community. The goals of the program are to help stabilize complex patients and prevent hospital readmissions.

The Clinics work closely with the Transitional Care Management and Hospital Case Management teams to ensure patients have a follow up appointment after their hospital stay within seven days, provide medical case management to patients who may need additional follow up, medication reconciliation, and transition to a permanent medical home.

The new clinic provides the following services:

  • Primary medical care
  • Medical case management
  • Medication reconciliation
  • Social work interventions
  • Transition to medical home

The Leesburg Clinic will be able to manage patients that have been discharged from Inova Loudoun Hospital and have no other medical home. Patients will now have a place to go that will assist them in managing their complex disease states until they can be transitioned to a permanent medical home.  This will not take the place of patients’ already existing primary care relationships or their choice of who to see in follow up.

Inova Juniper Program, Northern Virginia’s largest provider of HIV/AIDS care, opened a new location in Leesburg, VA, on October 1, 2012.  Since 1988, the Inova Juniper Program has offered all aspects of primary medical care and will do so at the new site.

The new clinic provides the following services:

  • Primary medical care
  • Diagnostic and lab services
  • Pregnancy management
  • Medication assistance
  • Nutrition counseling
  • Mental health therapy
  • Substance abuse counseling
  • Support groups and more

Inova Juniper Program provides care to more than 1,300 persons living with HIV/AIDS throughout Northern Virginia. Opening the Leesburg Clinic has provided services to those living with HIV/AIDS in the community. This has allowed patients to be seen in their neighborhoods and has allowed the Juniper Program to work closely with local providers.

    I am a pharmacist, currently working at the Inova Fairfax Hospital Medical Center campus. Are there any positions available for a pharmacist in Transitional Care Management services? I live in Sterling. A reply would be appreciated. Thank you.
